Q: Startup with flashing question mark folder

Hey guys, English is not my mother language so I have to apologize for my poor English first.

Okay then here's my question :

I got a flashing question mark folder when I restarted my macbook pro retina (13.3" 2015 late, OS X 10.11.4 beta 6) last week.

So I get into recovery HD to see whether if I can do a clean install to fix it, but I can't find my disk to reinstall OS X, there was nothing showed up.

Then I reboot my mac again and again, sometimes I can boot up normally.

But I can't found any thing wrong, it just worked as usual, until I restarted it again, it dead again.

I took it to the Apple authorized service provider to fix my mac. Yeah, there's no official Apple Store and genius bar in my country.

They thought my disk was dead, so they replaced a new disk for my, it took me a week to get my mac back.

It seemed work perfectly, so I restore my data back by Time machine, and update to 10.11.4 beta 7.

Yesterday the 10.11.4 came out, so I updated my mac to this version.

But when it rebooted during the installation, it showed the question mark folder again.

After a few try of rebooting, it completed the installation. then I tried to restart it to see if it's Okay or not.

Unfortunately, it came up with that folder again. So I turn it off.

After half hour I tried to boot it up again, it boots up.

Everything looked normal, I've checked the S.M.A.R.T by DriveDx and the data were all good.

I repair permission by DiskWarrior and it said no need to repair.

But I still get question mark folder with 1/10 chance when it startup.

I don't know what happened to my mac, is there anyone has any suggestion?
