How are the photos stored on your iPhone?

There are two kinds of photos that cannot be transferred by a USB connection.  You can only transfer photos that have been saved to the camera roll on the iPhone.

    1. Photos that have been synced to you iPhone using iTunes Photo Sync are not in the Camera Roll and cannot be transferred back to your Mac, unless you mail them, see:  Sync photos to your iPhone, iPad, and iPod touch in iTunes - Apple Support
    2. Photos that are in iCloud and not local on your iPhone cannot by transferred to your Mac by  a USB connection, only the photos downloaded to your iPhone from iCloud.  If you have iCloud Photo Library enabled on your iPhone, use iCloud Photo Library to transfer the photos to your Mac, or you may be importing a mess of duplicate photos with previews of different pixel sizes.
