The only concern I would have is how having the large copy of the screen helps without being able to actually touch the large screen. Having to look at the large screen and then look back at the small one and find the same spot to touch would be confusing to me.

Thanks for your proposal. However, yes, I activated what could help this friend (Voiceover, Siri, etc), things in Accessibility.
He just want to read SMS and mails. And few iBooks. Siri can be asked for...
He is also OK to manipulate his iPhone 6 Plus looking at a 32" screen.
I think the solution could be to forget Apple TV and connect directly with lightening cable interface (VGA, DVI, HDMI, we have a choice).
We'll be testing soon such connectivity and I'll keep you informed.
