Q: strange spotlight behavior 10.11.3 (15D21)

Spot light has gone bonkers or I don't understand how it works. Used to be just fine. For example, when I searched today for a document beginning with the capital letters DFG (I know I have many) an icon of my Contacts appears to the right of the search line. Many files beginning with DFG are not shown, even by scrolling up/down, but, if I double click "Show all in Finder", then I see many more items. I open one with MS Word. Close it. Quit Word. The try DFG search in spotlight again. As I type DFG it has the full name of the document i just opened with grayed out letters past DFG and an MS Word icon to the right of the search line. That document is the "Top Hit." I can't see any other documents in the list of which I have many that begin with DFG. If I type more, the MSW icon stays there and I get no more hits. Again if I double click "Show all in Finder", I see a lot of documents again.

 

Am I doing something wrong? I never had this icon show to the right of the search line before. Is this an enhancement? I used to see the full list when I typed the search into spotlight. No icons, just results.

 

If there is any detailed info about how Spotlight works, I would appreciate a link. Thank you all in advance.
