The back of Apple Watch may become scratched if debris is caught between the watch and the charger. Cosmetic damage (which includes scratches) is not covered under the One Year Limited Warranty.
To prevent damage to your Apple Watch and the charger, Apple advises that you keep both items clean.
Charger: disconnect the charger from your watch and from the power adapter, wipe the charging surface with a damp and nonabrasive cloth, then dry it with a nonabrasive and lint-free cloth. Do not use any cleaning products.
Apple Watch: turn your watch off (press and hold the side button, then drag the Power Off slider to the right), remove the band (by depressing the band release buttons), then wipe the watch with a nonabrasive, lint-free cloth. If necessary, you can lightly dampen the cloth with fresh water. Dry your watch with a nonabrasive, lint-free cloth.

I noticed recently that the center of my sensor was scuffed up also. I have been wearing my watch almost continually for nearly a year, so I first thought it was
jjust normal wear. I just received a replacement watch (for other reasons) from Apple, and then it dawned on me that the scratches were from cumulative
mmovements on the charger puck. So, for now, to prevent this new one from getting scratched I place a small piece of tissue between the puck and the watch.
i think I may wipe a thin coat of silicone across the charging surface of the puck or else get something similarly thin and soft to use instead of tissue, like one of those thin cleaning cloths. Hope this helps. -steve

You should not place anything between the back of the watch and the charger.
If you are concerned about potentially scratching the back of your watch, instead wipe the back of your watch before placing it on the charger - and also keep the charger clean.

It sounds like it could be so harmful. Could you please provide a little more information as to why
this should not be attempted. I'm sorry, but I really need to the ' why ' behind things.

Apple specifies that the back of the watch should be placed on the charger. The surfaces should be clean and dry and placed in direct contact with each other.
It is normal for the power adapter, charging cable and watch to become warm during charging. Charging should be carried out in a well-ventilated area, with neither device covered by anything (whether flammable or otherwise).
Rather than stating that foreign objects may be placed in between the charger and watch, Apple instead highlights the importance of removing the (very thin) plastic wrap from the charger. Some users have reported that accidentally leaving this on the charger has prevented proper charging and caused the watch to become abnormally warm.
Placing anything between the surfaces may also lead to the watch becoming misaligned during charging, with similar potential results.
Using charging solutions that do not meet Apple's performance standards (including interfering with the intended operation of the supplied accessories) could result in fire, electric shock, injury or damage to the watch and/or other property. Any such damage to the watch would also not be covered under warranty.
If you are concerned about potentially scratching the back of your watch, wipe your watch before placing it on the charger and also keep the charger clean.
