When you transfer photos from your Mac to an external drive, that does not remove them from iCloud. Those are just copies.

For as long as you want to keep the extra storage space. It's a recurring service, like any other monthly service, like Water or Power, or your Cable Tv bill.
You keep paying as long as you want to use it.

Copying files from your Computer to an external hard drive does not involve iCloud at all.
If the files you copy to an external drive exist in the iCloud folder on your Mac, deleting them from there after they are copied to the external drive will release the space used by them in iCloud.
If you stop paying the monthly fee, the extra space will go away, and anything on it with it. i.e No you don't keep your photos if you stop paying for the space they are occupying.
