It looks like a graphics card issue. Take it to any Apple Store for a free evaluation.

That photo gives every indication of a failing GPU. As this is a hardware issue, your choices are getting it fixed at your local AASP or replacing the computer. If you wan t to take it in for service at an AASP however you don't know of any in your area, please click https://locate.apple.com/country to begin finding one.

    Good luck!

We would have no idea of the cost to replace the GPU or the iMac. That is why you need to speak to an AASP to gather that information and then determine for yourself the path you want to take.

However considering the age of the computer (7+ years old) I would say it makes more sense to replace the computer than to repair it. Before you do, of course back up the computer using Time Machine or create a bootable clone. Then when you get a replacement Mac you can migrate to the new machine when you turn it on for the first time. Apple explains this in Move your content to a new Mac - Apple Support.

If you do decide to buy a replacement Mac, one thing to consider is one of Apple's refurbished machines. There is no downside to these, you can find them by looking at the bottom of Apple's home webpage and click Refurbished.
