If you have an Exchange (e.g. work) email account set up on the iPad, or have a profile installed, which could be forcing a passcode change.
If not try closing Safari : Re: Strange passcode dialog: feature or malware?

Nonetheless try resetting Safari. Close Safari completely from the app switcher window by double clicking the Home button and swiping up the Safari preview pane until it disappears from the screen. Then go to Settings > Safari > Clear History and Website Data. Then do a forced restart. Hold down the Home and Wake/Sleep buttons at the same time for about 15-20 seconds until the Apple logo appears.

Jun 14, 2016 3:12 PM in response to King_Penguinby Moheroy,Closing Safari does not tell you anything, those instructions in the link are just a fetish ritual and tell you nothing. If it is malware someone was able to hack into Apple iphone settings which is a pretty big deal.
When it happened to me it was when I first picked up the phone.

This happened to me about 6 months ago just change passcode from 4 digits to 6 digits
Its for better security .
No problems .
