I understand that you're not able to get your Mac to produce any audio aside from alerts. In the screenshots you've provided, it would appear that your Mac has detected that there is an Optical audio cable present. The steps outlined below detail what to do in such a situation.
- If the Internal Speakers option does not appear in the Output tab and your computer is not a Mac Pro, check that Digital Out is present as in the image below:
- Confirm again that all external audio devices and cables have been removed.
- Check to see if a red light is lit in the line-out port. If so, your Apple computer may need service.
- If you do not see a red light, proceed to step 5.

- If the issues persist, contact Apple support for further troubleshooting assistance.

Jan 5, 2016 8:36 AM in response to Allen Aby sraubenstine,I had the same issue. Digital out as the only option and internal speakers missing. I plugged in headphones, then unplugged them. The internal speakers returned.

Aug 5, 2016 5:12 PM in response to sraubenstineby nico_y_m,I tried that too, plugging my speakers back in, and then unplugging. Did not help. Now I'm on hold for Apple Support.
