1. Yes, both in OS X Recovery and Internet Recovery. dev/disk1 always formats, dev/disk0 will not. Even after swapping drives and first formatted drive now shows unreadable partition

Turns out the original SATA connector has failed. Put both drives on both cables onto both MB connectors last night and everything works except the "upper" SATA cable that came with the original HDD. Must have damaged it when disconnecting first time. I'll order a new cable and should be good to go. Followed advice from OWC to arrive at outcome.
