Q: Resetting my security questions & PW

I want to reset my Apple ID password but, when I tried to answer my security questions, a message box popped up saying that one or more of my answers was incorrect.  When I tried to use the Apple website to change my security questions, a message appeared saying something to the effect of "there is too little information to change your questions".

Can anyone please tell me how I can reset my password and security questions?  Any help here will be really appreciated.
