Do you have any strain on the cable? It could spread the contacts inside the plug over time. It is also possible that the connector on the Mac itself is damaged from this. make sure that your outlet is stable and its not losing connection there. I would try replacing the power chord (simplest way) and if that won't fix the problem, maybe bringing it to an Apple store might work out for you.
