iCloudTry logging out of iCloud and then back in again, then go back to the Manage Storage screen shown above to see if the 5GB option is checked when you initially go into the screen.

iCloudI can see the billing cycle, I am at the start of the month in the cycle.
I have logged out and back in again but the 'Done' button is still disabled. The blue tick is still on 5GB Free but all I can do is cancel, I cannot click 'Done' to confirm the change.

iCloudThere is a lock icon in the upper right corner of the screenshot you have posted. While this lock is active, the button will probably not work.

iCloudI'm a bit confused about your screenshot. I just noticed that the message at the top of the screen indicates that you need to choose a new option before your plan renews on 01/09/16. That implies that you are not currently subscribed to an upgraded iCloud storage plan at all, but that you are only using the 5GB free plan.
Can you clarify?
