Thank you for using the Apple Support Communities. I know having an issue with Calendar notifications on your iPhone is not what you expect. If I understand correctly, you are unable to get the notification for the Time to Leave for Calendar events, even though the Time to Leave notification for the same Calendar event works on your iPad. Based on that, I would use these steps to get the notification working on your iPhone again:
1. First, I would go on your iPhone to Settings > Notifications > Calendar and turn off Allow Notifications. Then restart your device as the shut down and start up process is important to keep the iOS software running properly. Then turn Allow Notifications back on and test this issue with both existing events and when adding new events.

2. If the issue persists, then I would suggest backing up and then reset all settings on your iPhone. Now, don’t worry, this will not affect any of your content. All of your contacts, pictures, music, and other stuff is all safe. What it will do is reset your custom settings back to the factory defaults. So, background pictures, sounds, fingerprints and stuff like that will have to be set back up again. Once that is done, then test this issue with both existing events and when adding new events.
