Was there a reason to erase the partition using Disk Utility? If you do not use the steps documented in Boot Camp: Remove Windows from your Mac, you will see this problem.
Run Disk Utility, reformat disk0s4 as a FAT32 partition, and run BC Assistant and click on Remove/Restore.
