Q: why does my os x keychain contain so many old passwords

I needed to find my password for itunes, since for some reason it did not sync to my IOS devices.  So I opened keychain access on my mac and tried the passwords listed for appleid.apple.com, id.apple.com, apple.stackexchange.com and finally idmsa.apple.com, which worked.  The first two contained old passwords, the stackexchange password was recent but was not the right one.  How do I clean this mess up?  Yes I can delete the first three, if they are no longer used by apple, but a better question is, how do I stay on top of this issue (for all sites), since this is not the first time I have run into it, and in some cases it is much worse.
