Q: iPhone6s - Multiple DCIM folders

Why does iOS (current v9.3.5) create so many DCIM/xxxApple file folders and how can I stop this? Some folders have 1 pic, others have many...

This is how iOS manages your photos.

You are not supposed to be accessing photos as if it were the Finder on a Mac.

All management of your photos should be done via the Photos app, or using the following steps.

Import photos and videos from your iPhone, iPad, or iPod touch - Apple Support
