To rule out a software issue, set ti back to factory settings, as described in this article and test the camera after that:
Use iTunes to restore your iOS device to factory settings - Apple Support
In case this does not work, the problem is hardware related and the phone has to be serviced by Apple.
Find an Apple Authorized Service Provider
Make an Apple Store appointment
There is a discussion about some users with a similar problem , however, I would not do that to my device.
