I see that you're posting today because your iPhone 5s isn't ringing or vibrating when an incoming call or message comes in. It's certainly important to be alerted when you receive a call or a message, so I can understand the concern here. I'm glad to help.
Is this occurring with all sounds from your iPhone's speaker or only with your ringtone and text tone? First, I'd recommend making sure that you have your device in ring mode by checking the silent switch on the side of your iPhone. Take a look at this overview of the Ring/Silent switch:

In ring mode, iPhone plays all sounds. In silent mode (switch shows orange), iPhone doesn’t ring or play alerts and other sound effects (but iPhone may still vibrate).

After you've confirmed that you have ring mode engaged, navigate to Settings > Sounds and make sure you have Vibrate on Ring and Vibrate on Silent both enabled. You can also check the ringer and alerts volume here to make sure it is turned up sufficiently.
Once you've completed these steps, try testing again to see if the problem persists. If so, I'd also recommend that you restart your iPhone using the steps found here:
