The Untitled partition is formatted as HFS, which is read-only from Windows. If you want to read-write, run the following command and post it's output.

    diskutil eraseVolume fat32 "SHARED" disk0s4

    Now test read-write capability.

Now can you see this partition in both OS X and Windows? If you can, try to write a file from one OS and update from the other and test.

Disk Utility seems to have overwritten your MBR when the "SHARED" volume was formatted as FAT32. Please post the output of

    sudo fdisk /dev/disk0

If you see an MBR which does not match 3 4 5 layout, can you recreate it and test?

As expected, can you recreate the MBR with 3 4 5 and test?

You, my friend, are a genius! Thank you for all your help - everything works including the previously unusable partition

Please remember to back up OS X, Windows and your SHARED partition regularly. Create a Windows System Restore point for future recovery.
