Sometimes it works ok for an afternoon or a day, but always returns ...
I just keep Terminal handy, and force re-appearance of the icon as needed - but there's gotta be a real fix for this..

The second half of the article details what is known about the problem in Leopard. If you (or some software) added a Spotlight Comment in GetInfo you need to remove the comment and then run the Terminal command, it should then stay visible when the visible bit is reset.
