keychain

Keychain suddenly stopped filling in some logins which before had functioned just fine, how can I fix this? They are listed in the keychain, and I have use first aid with no errors indicated.
Also, what is meaning of "passwords not saved"?

Launch Safari->Preferences->Autofill->ensure that all three options are selected. passwords not saved indicates that the site will not accept username/passwords from the keychain and you'll have to manually enter them. It's a security feature at most financial sites to preclude people from accessing them automatically. This is useful, especially if the machine's stolen or used surreptitiously.
