My iPhone only works on speakerphone

When I try to talk on the phone I can't hear anything and I can not be heard unless I use speaker phone. I have the volume turned all the way up. I have turned the phone off and back on again. Has this happened to anyone else? What can I do to fix this?

Macbook, Mac OS X (10.4.10)

Try plugging in and unplugging the headphones about 10 times.

The suggestion that Ansuz82 gave is one of the suggestions discussed in this article:

If connected and disconnected headphones several times does not resolve it, some of the other steps listed in the article may be of some assistance.
