As the prior poster indicated, it is necessary to install the Palm Desktop - currently at revision 4.2.1 D for the Treo 650 - in order to synchronize using iSync.
In a quick summary, the process is this:
* install the Palm Desktop provided with your Sprint PCS-supplied Treo 650 on CD-ROM
* update this installation by running the Palm Desktop 4.2.1 revision D installer
* pair your smartphone and your Macintosh and
i confirm this pairing
using the instructions provided by Palm referred to alone
* synchronize at least once with The Palm Desktop
* enable synchronization in iSync, being sure to configure its preferences to support the Sync Status menu and HotSync
* add your device using iSync's Enable Palm OS Syncing
option
* configure your device using the drop-down configuration pane which appears when your select your smartphone in the main window of iSync
* synchronize using iSync
If you run into ANY problems at all, stop and write for help, as you can inadvertently and nearly irretrievably damage your configuration, necessitating many, many steps to correct the problems you create. Once it is set up properly you should experience few problems, but sloppy programming and oversights by both Palm and Apple make it an overly complicated process, fraught with the potential for error.

Post any questions you have

attempting to perform these steps - and don't skip any of them - and you should enjoy a reasonably trouble-free installation and synchronization out of the box.
