questionnaire? iwork, pdf or iweb

I have a new company product that I am going to send out a questionnaire to customers, buyers and ski companies. Our product is a new rotational binding that allows the rider to choose his/her stance on the board and does not require rider to dismount or take board off foot.
I want to find out what aspects they find most appealing and if they would like to learn more. So I would like to design an email or make a web page that they can access to give me their ideas and feeling on product
IE to give them a short multiple choice scale 1-10 usability and functions
such as on a scale of 1-10 do you feel this would help in comfort of going through lift line in foot forward position- (then give them points to choose)
I am not sure how to build this - how to get information back to me and how to also give them just an area for their own ideas.
Anybody made one of these or know where I should turn to get help building it.

None of those tools give any easy solution for questionnaires, unless you are willing to print them out and then read the printed answers.

Adobe Acrobat has one solution, but it needs backup from a web server to be efficient. Quick googling send me to http://www.surveyspace.net/ , which I never have used, but it is possible that they can do something you would want to use. If not, I suggest some further googling.

I think your best bet would be to simply make and post a survey for them to take. Then the survey will automatically gather all of the data and present it in easy graph form for you to look over. There is one such service which is free for use on the net, but I think they limit your questions to 10 in number. You could of course start additional surveys if you wanted to, or pay for their service and include all of your questions in one survey. Usually 10 questions are enough if you plan out the survey ahead of time. All you'd need to do would be to send them a link to the survey and you'd be all set. You could include that link in any of the products that you asked about. Off hand I don't have the link to the site, BUT I'd be more than happy to do some research for you and post the link later if that would help you. Just let me know 🙂
