Really need some help. I've lost my Wufoo form on my site

I created a wufoo form about a year ago and in the last week it has disappeared off my site. My problem is I can't figure out how to get it back there. The form code is already created but I'm missing the part where I should be copying it into a blank HTML page. I've copied it but when I publish the site there's no form. Can someone explain how to copy it so it will post? It's been so long since I created it and published it I can't remember.
