It is possible that a refurbished unit may come with some minor cosmetic defects. Apple cleans all units before repackaging, but it's possible that there could be some light scratches or mars. Apple to my knowledge doesn't guarantee the cosmetic quality of a refurbished unit, only that it will function correctly. Most units will come without any scratches or mars, but there's no way to be absolutely certain.
We have three Mac refurbs bought directly from Apple and each has arrived in pristine condition. Furthermore, all three have been real workhorses that never complain.
The first refurb I bought predated the Web Store and required a phone call. I asked the agent if these Macs were returned as defective and rehabbed, She said that is possible but rare; by far the majority were overstock units on hand when a new model released.
I have purchased 2 refurbished Mac's, an iBook and now a MBP and both have been great. I have had no complaints about the cosmetics on either one, they looked brand new. I recommend refurbs to all my friends.
