Resetting Connect to Server window

Hi,
How do I reset the "Connect to Server" window to come up again?

Normally when we connect to server, the window comes up and theres a tickbox about Clear Password. One of our staff unticked the box and now we cant connect the machine to the server. We just need to tick that box if only we can get the window to come up.
