What causes the PUK prompt to show on the iphone?

I work at an at&t store and a few customers came in, with a message on their iphone asking them to put in their PUK number, i want to know what is causing that?

Hey firestar87,

This happens when the incorrect SIM pin is entered multiple times, it can happen with any cell phone.

+"You can enter up to 3 incorrect answers for the SIM PIN before iPhone asks you to enter the PUK code. To get the PUK code, contact your mobile service provider. Note: If you enter the incorrect PUK code too many times (10 incorrect guesses), the SIM may become permanently locked and you'll need to replace the SIM.+

+Note that the SIM PIN is separate from the Passcode Lock (Settings > General > Passcode Lock) which comes up when waking from sleep and turning on."+

The Pin Unlock Key is requested after the SIM PIN is entered incorrectly too many times (I think three or four).

Generally, only customer service can look up the PUK, and entering the PUK incorrectly too many times will permanently destroy the SIM card on most phones. (I don't know specifically if this is how the iPhone works, but that's the standard.)
