Exchange Calendars Synchronization Issues Galore

Events added via the phone are not showing up in exchange either.

I notice a lot of warnings in the exchange windows event log on the exchange server from my iphone
does not appear to be an iphone issue
will post details when i resolve the issue

Here's the exception:
An exception occurred and was handled by Exchange ActiveSync. This may have been caused by an outdated or corrupted Exchange ActiveSync device partnership. This can occur if a user tries to modify the same item from multiple computers. If this is the case, Exchange ActiveSync will re-create the partnership with the device. Items will be updated at the next synchronization.

Delete the active sync virtual directory and re-create it with the command shell to resolve this issue.
