Exchange ActiveSync error 3005 - I'm about to slit my throat.

I've been wrestling with my new iPhone 3G and Exchange for days; I have tried everything and it's still not working. I wonder if any kind souls on this board could lend a hand. Here's some info:

Windows Server 2003 SP2
Exchange 2003 SP2
iPhone version 2.0.1 (5B108)

Everytime I try to sync up to the exchange server, it triggers the following event in the Windows Event Viewer:

Event Type: Error
Event Source: Server ActiveSync
Event Category: None
Event ID: 3005
Date: 8/9/2008
Time: 8:13:03 AM
User: DI\swieser
Computer: DISERVER
Description:
Unexpected Exchange mailbox Server error: Server: [diserver.DI.local] User: [swieser@designandintegration.com] HTTP status code: [501]. Verify that the Exchange mailbox Server is working correctly.

There is nothing I can do to get rid of this error. I have tried all of the suggestions that Microsoft's help file has for this particular error. Additionally, I have tried all of the following Microsoft KB articles:

http://support.microsoft.com/kb/898131 (add host header for local netBIOS name)
http://support.microsoft.com/kb/817379 (method 2; add new exchange-oma virtual server)
http://support.microsoft.com/kb/883380 (method 2; reset exchange virtual directories)

- I have a valid SSL cert (not self-signed); OWA works fine in https or http.
- When I go to mydomain.com/Microsoft-Server-Activesync, I get an HTTP 501 error (not implemented), which is expected (or so I've read).
- When I go to mydomain.com/oma, it tells me the device type I'm using is not supported (since I'm using a PC), but otherwise let's me browse through my email in a text interface. This is also to be expected (or so I've read).
- I've tried putting the domain in my username and leaving it out, and I've tried deleting and recreating the mail configuration instead of just modifying it. Everytime, the phone says that my exchange account has been successfully verified, and that I have successfully configured sync with exchange. But, of course, no mail, no calendar, no contacts. Just a whole lot of 3005 errors in the event viewer.
- I've tried turning on and off forms-based authentication in the exchange server. SSL is configured but not required. I've restarted the IIS services after each change I make.

I honestly have no idea what to try. I'm tempted to copy & paste all of the details about my exchange configuration here, but I fear that no one would actually read this thread if I did. Does anyone have any specific info on what this 3005 error actually is, and what I can do to get rid of it?

I've been having the same problem as well, but I get a server error when I access the OMA directories. I've rebuilt the exchange VDs twice now; same results. OWA works fine, both http and https. I have a valid, signed certificate.

My guess is my problem lies in the OMA config...but I can access my email, calendars, and address books, but push email is what generates the error. If I find a fix, I will post it here and my blog.
