Firewire Mixer intermittently works and sometimes crashes Sounds Preference

I have a Phonic Firewire Mixer Helix Board 24 MKII (HB24 FW MKII) I cannot get it to work properly or be correctly recognized on my IMAC. In system profiler everything looks ok and shows the correct info listed here;
Maximum Speed: Up to 400 Mb/sec

HB24 FW MKII:

Manufacturer: Phonic
Model: 0x70000
GUID: 0x14960007000000
Maximum Speed: Up to 400 Mb/sec
Connection Speed: Up to 400 Mb/sec
Sub-units:
HB24 FW MKII Unit:
Unit Software Version: 0x10001
Unit Spec ID: 0xA02D

However in Sounds in System Properties the firewire does not show up? I have to continue to remove the cable and restart the Mixer and maybe it will show up in Sounds input and output. I select it and it crashes now. I have to do this before I can go to Audio/Midi Setup and find the device. Also I noticed that another device is listed in Audio Devices in properties for: the apple logo by each setting and the default is checked however at the bottom is listed Phonic Helix 18 Channel Firewire? This is not my mixer.

1) How do I remove this wrong property and why is that showing up? Is this the reason I am having trouble with my Helix 24 MKII?

Any help is greatly appreciated
