If I give an album to someone as a gift through iTunes and he already has it, can he return it and get something else through iTunes?

Sorry but no. Gifts cannot be returned or exchanged.
Will the gift recipient be able to refuse the gift or to pick something different? My concern in gifting an album is that the recipient may already own the album in CD format or a movie in DVD or Blu-ray.
Thankfully, in the physical world, gift recipients can typically exchange a CD or movie for something different if they already own it -- as long as they don't open it. There ought to be something similar on iTunes, is there not?

You then might rather want to give a gift certificate rather than a specific item. That way the recipient can choose whatever he/she wants.

No, there's not, since there's no way to "return" a digital download as is possible with a physical item. The download store has no way to verify that the item has been deleted from the system. As to being able to just exchange an item for another before downloading, it's probably too complicated technically to justify the work involved in making that possible given the very low profit involved.

I disagree. First of all, there is no interest in the customer in that response, but secondly and more importantly the main fear in gifting music is to gift something someone already owns. Implementing a feature like this would increase the number of people gifting music and profits in turn.
I know there are always gift certificates, but those just aren't personal enough.
