No matter how many times I remove the Bluetooth icon from the menubar or change preferences it keeps reappearing after reboots, even awakening from sleep.
You probably have Bluetooth configured to run at start up.
To change this open up the Bluetooth preferences. In the bottom right hand corn there is an Advanced settings tab. If you click on that there will be options such as "Open Bluetooth at Startup when No Devices Present." If you uncheck that it should disappear for good.
Well even if you uncheck the "show bluetooth in menu bar" unchecked and leave "start bluetooth on startup" checked it will just reopen in the menu bar. Did you try unchecking that option and seeing how it responds?
