Gift song or album - what if the recipient already has it?

I want to gift a friend a song or album from iTunes.
What happens if they already have what I gift them?
Do they get the option to exchange it for credit?
Or are they stuck with 2 copies of it?

Sorry but no. It's not possible to exchange a gifted item for credit or another item.

Yes, that would be the case.

It would therefore really be best to confirm that your friend doesn't have the song/album before you make the gift. Or if that's not possible/desirable, buy your friend a gift certificate instead, something that wouldn't be practical for just a song since the minimum amount is, I believe, $10.00 (US), but would be practical if you wanted to give an album.
