There seems to be a random issue with this. Interestingly, if you were to remove the Exchange account and re-add it, the entries all fill in fine. We are using Exchange 2003 SP 2 fully patched. This has happened with 2.2.1 and 3.0 on the iPhone. I'll have to see if the 3.1 beta is similar.
I'm having the same issue. Certain events show up on my outlook calendar on my computer, but don't show up on my iPhone. I have 3 meetings scheduled today, but only two show up on my phone. I went into the settings and turned the exchange calendar off and then turned it back on and everything seemed to sync up ok. I'm not sure what the glitch is, but turning the calendars on and off seems to fix it.
I don't, as this has "fixed" the issue for me everytime. I am now on a beta of the firmware, so I can't test anything further with 3.0. Sorry I'm not of much more help.
We have the same problem. Tested on multiple iPhone 3Gs running 3.0 iPhone software.
We have narrowed the issue down to:
1) Accept meeting on iPhone - works fine, shows up both on exchange and on iphone
2) Accept meeting on exchange (via Outlook, etc) - shows up in outlook, does not show up on iPhone.
3) If you delete the exchange account, re-set it up from scratch. It will resync the calendar fine. But, it's just a short term fix. The next time you accept a meeting from your computer - it will not sync the meeting up to the iPhone.
This has been tested multiple times, on multiple phones and exchange accounts.
We have several 2G phones that have been using the Exchange system for years and they worked fine until the 3.0 software hit. (Same bugs above.)
At this time, all signs point to a 3.0 software bug. I hope someone official at Apple is aware of this? Has someone officially submitted it as a bug? (Where do you do that?)
I'm on hold with Apple Support right now about this. We have multiple users in our environment experiencing the same thing with both 2G and 3G phones but all with 3.0 software.
For what it's worth, I'm also experiencing this issue (appointments not syncing to iPhone when accepted via Outlook) with the following setup:
iPhone 3GS v3.0, Windows XP, Outlook 2003, Exchange 2003
I'm fairly certain that calender items synced correctly when accepting via Outlook on my original iPhone with v2.2.x.

Beyond disabling/enabling the Exchange account on iPhone, I've taken to editing the meeting events in my Outlook calendar by simply adding a couple of newlines (opening the calendar event, scrolling down and pressing "enter" several times) which causes the event to have to be "saved" again. This save operation has the net effect of forcing it to appear in iPhone. Again, not solution at all, but a terrible workaround for an annoying problem in the short term.
We are experiencing a very similar issue. We bought several units to test in our environment as a possible replacement for several hundred Blackberry/BES users. OS 2 worked great, but as soon as we went to OS 3 we can not get the Calendar app to function properly with Exchange 2003.
There are two issues:
1) When a user accepts from the iPhone and multiple contacts are on the invitee list, the user with the first name (alphabetically) in the list is shown as the acceptor/decliner for the meeting announcement.
2) Meeting announcements are not showing up properly on the iPhone when accepted on the PC
We had zero issues with 2.0, we have this issue on all phones upgraded to 3.0 and on any 3GS units.
Very disappointing that we have not been able to get any info from Apple support other than "slick your phone and try again," or "it must be an issue with Microsoft or Exchange, sorry."
I have had a slightly different but other calendar sync issue on iPhone 3GS. I have a calendar entry that was changed from all-day to timed, and the iPhone just won't seem to pick it up right.
Having all the same issues listed in this string. I've also noticed specifically the problem of accepting an appnt on the computer before it has hit my phone as a meeting request (like when I'm sitting right there at my computer). Then I get the appnt on my computer and a MESSAGE pops up on the phone (like a reminder), but no appnt (nor any request) is put in the phone's calendar.
Wish we could just blame it all on Microsoft/Vista/Exchange... but Apple needs to step up and take care of this one.
