Unknown error 13109 & Vista Explorer Crashes

Problem 1: I'm having major problems with my iPhone 3Gs as everytime I plug it in to my PC (Vista) Windows explorer crashes. It happens just as iTunes picks up my iPhone.

Problem 2: I'm also getting 'An unknow error has occurred (13019).' Steps to try to resolve this was to uncheck all syncs, and then sync. Then gradually check the boxes to sync stuff as I go along. But I'm still getting this error message.

To be honest I'm seriously considering formatting my hardrive again, which by the way I did a few weeks ago which seems to have caused all these errors...

Any help would be greatly appreciated.

First I'd read what it says here:

Unless your Vista machine is otherwise misbehaving (BSOD) I would format the HDD only as a last resort.

A software conflict could be causing that, too.
