Headphone issues?

I have encounter this issue. If I mute the speaker, then I plug in the headphone, the speaker will automatically unmute. But, headphone has no sound coming out from it. No matter how I volume up and down, it won't work. Only will work if I mute and unmute again, then the sound will appear. Does it related to the OS issue or hardware?
