I was confused as you are, and after using many shopping list apps i decided to use the KShop for one reason it's simplicity and friendly user interface.

As Al said, i'm using KShop as well and i think if you want shopping list app that manage your shoppings this is your app.
I was using KShop for my Christmas gifts shopping and because of this simple app i do it in a half time than last year.

I have been using this new free shopping list app from the Make it @home website. It's been working great so far since its so interactive with the website which makes it easier for me since I'm on the computer all day anyways and later when I come home from work my grocery list is ready on my iphone to use...

I feel like this one has great features and it has an awesome website with recipes anyways so why not try it out. Let me know if it works well for you or not.

UltraList Plus

Used a grocery store list that even had a feature that had a database of bar code numbers, but ultimately I couldn't organize things the way I liked. UltraList (forgot what the extra money for the "Plus" version bought me) allows me to sort items by two criteria: isle number at the grocery store and location in my house. Took some work to set everything up but I love it now. Asking my wife on Saturday, "OK if I go to the grocery store?"
