Scan without printing the document?

A couple of other thoughts. Try using Adobe [Acrobat|http://www.adobe.com/support/downloads/product.jsp?platform=macintosh& product=10] instead of Preview, it often works on filling out forms when Preview doesn't. Also there is a pretty cool Mac program called [PDFPen|http://www.smileonmymac.com/PDFpen/index.html]. Finally you could "Print" your PDF to a PDF using whatever application you want, including Preview (in the Print dialog, click on PDF, and then select Save As PDF...), I've also had success editing PDF's that I printed from PDF's that I couldn't edit otherwise.

Why would you scan what is already a file? The whole point of scanning is to turn a paper document into a file, and your pdf is already a file. if you need to convert it to another format such as JPEG then you may be able to do this in Preview, I'm not sure, but you can certainly do it in GraphicConverter.

You could probably manage this by converting it to a PICT in something like GraphicConverter, and then overlaying text boxes in the appropriate places and then resaving it as a png - but frankly by the time you've done all that you might probably just as well scan it and fill it out with a real pen. If you have Pages you might be able to just drag the file in and then overlay text boxes - I can't check at the moment whether Pages can do this with pdfs.
