Select the photo, click print, select the printer and paper size - select custom print size and set to 2" x 2" - click customize and click the settings icon (looks like a gear in the tool tray) and select print multiple photos per page (the preview should show your print layout) and click print - select any printer specific options and click print to print.
Note that the US passport agency is very picky - I have found it worthwhile to go to a place that does passport photos or the post office for my photos

in iphoto select photo and then "print". in print window select custom paper size and input required photo size (2x2) . Then do not print but save as pdf. now we have new photo with req. physical size (for example 2x2). now it can be inserted in new Pages document as many times as needed and printed (on any paper size.). or exported into different format (for example jpg). that what i did when i was in need. in Pages Shift-cmd-P and select paper size or make your own (like 4X6)
Not easiest way, but "it just works"
