Templates for Pages...

I'm in something of a time crunch and I need to create printable petition using the Pages program in iWork. I don't have the knowledge or time to create one and was wondering if anyone knew of any petition templates that exist? I've looked around the web and found nothing. I'd prefer a free template but may be willing to pay if there's no other choice. I looked at Jumsoft's offerings but found no specific mention of a petition template. Any help would be greatly appreciated.
