How do I embed a form into an email?

I am trying to figure out how to embed an art approval request form into an email. When art is sent to a customer, I would like for them to be able to reply with the form embedded into the email to show that they approve the art. Also is there a way to set something like this up where the customer could add their comments, and etc to the form in the email?
