I just upgraded to the iPhone 4, and when I set the time to "automatic," it's off by 3-5 minutes. I've double checked multiple known time sources, and it's flat wrong. I've reset the phone multiple times as well and returned the settings to original OEM. Nothing.
I just set the time manually, but has anyone else had this issue?

I've had that issue for a brief while. The cell tower times could have been off. Cell towers adjust your clock automatically as it has been with normal phones on Edge/3G. If the cell towers are off by a few seconds or minutes, it can offset your phone as it adjusts by the closest tower.
Restored as a new iPhone may have fixed it.
The cell tower times being off would be my first guess, but when I have a iPhone 4 and a 3G sitting next to each other, the times are several minutes apart.
You wouldn't think that the iPhone 4 would connect any differently than other phones to the network but, hey, what do I know?
Go to settings->General->Date & Time
Turn off "set automatically"
Set your time to something very different than current time
Hit the home button
Go back to settings->General->Date & Time
Turn on "set automatically"
I have this problem, as well. My old 3G matches all other devices I have in terms of time sync, but my iPhone 4 is off by at least 45 seconds. I don't think the towers are the source of this issue. Who knows when Apple will get around to addressing this issue? Perhaps the upcoming firmware update that's supposedly in the works will address the problem.
Yeah its funny Because My girlfriend has the 3gs and when she sends me a text message if its been long enough above the message it will say the time she sent it and it will be ahead of the time on my phone on the time its like shes in the future and im in the past .
I, too, had this issue. After researching and trying many things (and going around several days with automatic adjustment turned off), I finally stumbled on a post where someone said to try a Network Reset. This appears to have fixed the issue for me.
I am also in DFW and my iPhone 4 is consistently off by 4 minutes when set to automatic. My wife has an iPhone 3G that always keeps perfect time. When both iPhones are side by side, connected to the same tower on 3G, they always read 4 minutes apart. This is not a "losing time" issue -- the time is NEVER correct on the iPhone 4, and the amount of time the iPhone 4 is off by is constant. I have tried the network reset, as suggested here, but the iPhone 4 time is still off by 4 minutes.
This doesn't happen when it "sleeps." It's continuously off by 3-5 minutes. I'll try the 3G off method and see what that gives me, though that's not ideal.
I too am having the iphone4 off by 4min. I went to the Apple store, they replaced it, i went home and restored as the Apple tech advised, I still have the same issue. Apple is blaming A&TT, I am on the phone with them now, A&TT has done everything possible and no fix. Now, I have Apple tech on the phone (through A&TT) He is saying it is a corrupt file when syncing the phone, were doing it now (ps advise them to call you back phone drops calls when syncing). I took over on my own from here (no call back) the tech advised to restore as NEW IPHONE (first make sure all sync are check in each tab click apply) when done click restore, restore as NEW (do not touch the iphone not even to unlock the screen) it will sync what you have on your Mac. Ta-da! All fixed, my time is accurate. The issue is a sync issue from a 3g to the iPhone 4. I am Happy Again! ~Vincent~
