Up date my iphone to ios 12.1.1 and apple watch 4 to 5.1.2 but I see no ECG app in my Heath app or under the heart app

I see no ECG app on either my iPhone or Apple Watch version 4. Both have been updated to the latest software. So I have noting to set up.

Thanks for asking about the new ECG feature.

Check here for availability in your region: ECG

ECG will not work on beta versions of watchOS or iOS and requires the current version of iOS and watchOS.
