I have an iPhone 6 on iOS 12.1.3 and my iMessages are only sending from my email instead of my phone number, so if I text someone when I have WiFi versus if I text someone via SMS, they would be receiving messages from two separate contacts.

Go to Settings/Messages. Tap on Send & Receive. In Start New Conversations From check your phone number.
Then iMessage shouldn't be working at all. Yet you say it is. Where are you in the world (country), and who is your carrier?
I also forgot to add that whenever I do go to settings, it says activation unsuccessful. I've tried logging in and out of my Apple ID, restarted my phone and resetting my network settings.
My iMessage only works if it sends from my email. I'm in Canada and with Telus.
