You can downgrade iTunes if you're not using an iOS device that requires the absolute newest.
Apple removed app management starting with iTunes 12.7. Read the second document on various ways to do this including using an older iTunes and manually installing ones already on your computer.

You used to be able to organize them on a device while using iTunes on a computer. Are you thinking of that? Apple removed that feature a year ago.
