I'd like to purchase the apple watch 4.0 GPS only, however, I'd like to be able to use the fall detection option. Would i have to purchase a GPS+ cellular watch instead? Does fall detection require either cellular or wifi connection, for it to work?
Fall detection definitely works on an Watch GPS Series 4. Since the watch does not have a cellular connection it must be connected to your iPhone in order to reach emergency services. As long as the Watch is connected to your iPhone you're good to og.

Fall detection requires access to your iPhone and that it’s near by to call which is connected via Bluetooth.
To reinforce what seanfromllanelli said. I have a GPS only Apple Watch, and Fall Detection has triggered, twice. Granted it triggered when I was shoveling snow and chopping at an icy bit, but it still functioned.
