Here is a list of countries that now have the ECG app;
I have the same problem.
I had an Apple Watch 3 until yesterday, when I bought the Apple Watch 4.
My Health app was already set up, and I got the notification a while ago to say that I could set up the heart rate notifications, which I did. I did not receive a notification to set up the ECG app because it was not available to me on Apple Watch 3.
Now that I have the Watch 4, I want to set it up.
About me:
Can anyone explain how to at least get the ECG app on the watch?
I have exactly the same issue, I am in the UK, Watch series 4 A1978, updated to 5.2 and iPhone XS max updated to 12.2, still no ECG app, also when I open the Heart settings in the Apple Watch app, it shows only high HR and Low HR settings, no ECG and not even Irregular Rythm settings which shows when my series is connected to my phone, it even says: ‘Those features are not available for all watches paired with this phone, which is super strange.
I have the same problem. Got 5.2 and 12.2 installed but no ECG app in watch. Though..... Bought my watch in China (use it in Sweden). ECG app not introduced in China yet. Saw attached info and called the support. They said that it will work when app is introcuced in China, but they could not say when. Sad. Asked them to change region (sales region). But that was not possible. Sad. Bad.
I am in Belgium and have the same issue 12.2 iOS and 5.2 WatchOS. I know it was launched in Belgium on March 27because I can see it in the Health app under heart. When I am on Watch I can see the heart app setting is updated in settings but I cannot see the ECG app. My location is Belgium and I did go through the set up on iPhone date of birth, all set up screens,......
Any help welcome
I sorry to say you are wrong. As I have it and live in the UK. UK is short for United Kingdom.

It’s not available in the .UK
yes nothing there, the ECG list is empty (0) and nothing happens when I open it.
also just to complete the picture, my phone is on 12.2, I also tried hard resting my phone and series 4 watch and starting over as new, still doesn’t work !!!
I'm having the same issue too. This is a UK series 4 Apple Watch, being used in UK and I'm over 22 as well. I'm on the cellular model via EE and the pain of associating an e-sim with a phone account again is putting me off from doing the full wipe and restore that people are suggesting.
Hi! I have the A1978 bought in Canada but I live in Spain...hmm...I'm starting to think that this might be the problem?? As the ECG is not available in Canada and the watch is like region locked?
Thanks!
Thanks. It would as the update to 5.2 was what brought the ecg function to the Watch. So that fine if you live in the country that has it. Don’t help any other person that lives outside those countries.
Have you checked your Health App?
I'm guessing that you have read this, Taking an ECG with the ECG app on Apple Watch Series 4 - Apple Support
Don't go to Heart in Watch App, go to the Health app, then tap on Heart there.
For the first step, please see Apple’s setup instructions in Taking an ECG with the ECG app on Apple Watch Series 4 - Apple Support.
Yes all those places, like one of the other users when I go to the ECG menu in the health app there is no option to activate
I can't see the ECG app
