I have the Apple Watch 4 and just updated to IOS 12.2 and have WatchOS5.2. Why do I not have the ECG app on my watch? I am in the UK and over 22.

Shame on you! Lol easy mistake to make. Yes only got the update last week 27th March. Still people having issues getting it working on in UK I believe it to do with the model number.
I need my eyes checked!

I take it you brought the Watch in the UK? If so call Apple support or visit your local Apple Store for further assistance
Umm UK is on that list AND the list supplied by Apple.

What is the model of your Watch it starts with A**** like A1114
It's A2008
Then you should have it working. Have you checked your health app? And looked at the heart settings? Also check your Watch heart app.
I have the same issue as well, no way of getting the ECG app to show up...I have tried all the solutions, unpariring, pairing again, resetting my iphone, nothing it just does not show up...
Which model do you have ? And what region or country are you in now?
